The results of the phase 2 trial with Haart and Remune. As I suggested previously the upfront HAART therapy is IMO not optimal therapy. The addition of Remune to one arm is suggestive that the drug is active. The problems the numbers are small and the upfront therapy is not the most potent combination. The question if Remune is capable of boosting specific T cell responses seeems based on early small numbers, none the less is encouraging. As an infectious disease doctor the problem we face now is durability. I have been amazed at the clinical and [lab marker response viral load CD4 ct] that we are capable of acheiving in both the naaive and heavily pretreated patients. My concern now is even with good patient compliance given the viral replication and high mutation rate of the virus that even HAART will not contol the virus with time. If in fact the addition of Remune to therapy is able to boost the patients immune response then the prospects for extended durability of existing anti-virals may well be prolonged. While this is a novel approach to stimulate the patients immune system it is so far promising. Time will tell. As the weeks go by the data even in small numbers becomes more meaningful. Bottom line too early to break out the champagne but if the trend hold Remune may make Viracept success seem trivial. More hope for pwa
