If I understand you correctly, the caching part is essentially the same, but it's the delivery of the cached content that differs. Second tier cable ISPs (i.e. ISP Channel, HSANet) are caching at the headend, and I assume the result is approximately the same as ATHM.

Boy is that an understatement. Unfortunately it seems to be getting worse. Both parties are equally to blame. Wish more people realized it.
