FWIW, I've been using ADSL in a business location near my home (my block can't get it, dammit,<g> for reasons they still haven't explained to me) and it's all its cracked up to be. I consistently see 80KB/sec during busy times, and around 120-150KB/sec during off-times. Considering the best you can get with 56K and V.90 is 5.4-6.0KB/sec, I'm seeing between 15-30 times the speed of 56K with ADSL.
I don't think you can pick your backbone though, unless things are different elsewhere. Here, with BellSouth, they become your ISP, with whatever backbone they use in that specific area. (not UUNET here, mainly ALERNET). Price is quite reasonable, $60.00 for the line, including ISP, per month. It actually drops to $50/month, if you subscribe to certain services as part of your local calling plan.
