Different funds have different policies. I think most of the index funds hold a position in all the stocks but weight the performers more heavily. The index is the benchmark to beat.

The statement doesn't say if the "appropriately weighted" is the S&P weighting or a weighting they determine within the fund. I gave a presentation to fund managers and I don't recall any hands going up when I asked who emulated the S&P weighting and the talk around the coffee machine led me to believe they use their own weighting.
