If you look at a chart of daily hi-lo-close, the gap is created when the low of a certain day was higher than the high of the day before (a gap on the up side) or just the reverse for the gap to the down side.
Now, gaps don't get filled 100% of the time. for instance , Intel has a gap from 88 to 92 made back in late sept or early oct....but it was justified because the market was undervaluing it severely back then.

lombard.com is a great site that gives you the capability of charting on an intraday or historical basis.
