Sowbug, for the top two/three list I'd submit the grammatical horror that I keep hearing and that sets my teeth on edge: "If you have any questions, please do not hesitate to ask John or myself." It's a cheap attempt to avoid choosing "I" or "me" and it's always, always, always wrong.
