Take the 640k deal. The higher rates won't get you much more observable performance unless you have specific broadband sources you downstream a lot. 640k is more than 10x faster than dial-up ... you'll be net- and server-contrained at 640k ... 7mbps would just let you wait faster. The numbers you've quoted are downstream rates ... also pay attention to the upstream rate if you upload a lot.

You'll be getting a Westell CAP modem. The new standard is DMT ... BA will be switching over this summer. DMT is somewhat more robust ... better able to handle noisy lines at length.

Your performance will be getting close to the T1 range, which is a revelation if you haven't experienced it.

I'm having 768k/768k DSL installed on the 19th ... $80/mon from GTE in Seattle area ... $99 total installation.
