Most of it was last summer and early fall so conditions may have changed but my recollection was it happening while trying to sell into ISLD bids on MSFT and INTC. They were showing the inside and right size but I'd get a locked/cross rejection every time I did it. Usually I'd be following the market down and finally get out through ARCA or when the market bounced, causing a few ticks of slippage.

As far as anyone could figure, there was likely a traffic jam of orders hitting the queue along with mine and the ISLD info was not to be trusted.
