John, I would suggest you call Iomega Warranty Support:

1-888-4IOMEGA

I don't know about Jaz disks, but a few in my department had faulty Zip disks (all purchased at the same time, about 2 years ago, they died upon doing a "long format" in the Zip drive, and Gibson's TIP program declared them unrecoverable). I called this number to ask about the warranty, andwas told they had a "limited lifetime warranty." When I asked what exactly that meant, the Iomega person said, "If they go bad, we'll replace them." I couldn't figure out what the "limited" part meant at that point, but she sent 5 Zip disks to replace the ones (all of them) that had died, and they were here within a week.

I would hope that a similar policy would be in place for Jaz disks.
