I emailed Qwest:

"I was told Qwest will waive the $4.95 monthly fee for Qwest stock holders. If so, I am a stock holder and would like my monthly service fee waived as well."

Got this response from Qwest regarding the monthly fee:

Jay,
That was a promotion from LCI before The Qwest merger, and is currently not available.
