The concept of urinary cytokines as markers for the intravesical inflammatory response has generated another active area of research. IL-1b, the soluble fraction of the first cytokine produced in the immune recognition cascade, was found to be significantly elevated in urine from patients with bacterial cystitis, compared with patients with IC and control groups. These investigators concluded that IL-lb may be used as a negative marker to exclude low-grade bacterial infection in a patient suspected of having IC. Lotz and associates found that IL-6 was significantly elevated in urine samples from patients with IC. In the same study, it appeared that higher levels of IL-6 correlated positively with pain scores. Both in situ hybridization analyses and samples from ureteral urine confirmed that IL-6 measured in voided urine was synthesized in the bladder. However, 2 other studies disputed these findings by showing detectable levels of urinary IL-6 in a small number of IC patients, and no expression of mRNA for IL-6 in IC bladder biopsies.
