I think the most viable form would be something akin to a diving mask with a pair of lightweight headphones. A helmet just won't cut it. I can already see the lawsuits from neck strain, bumping into walls, being laughed at by friends and relatives. :)

It definitely has to be 3D so the eyes don't have to focus on something and inch or two away, otherwise you're going to have eyestrain and headaches.

A great first step for me would be a monocle which could track the eyeball in relation to the monitor so an aiming reticle could follow my eye. This isn't too difficult. The monocle must be able to tell where it is pointed in relation to a reference on the screen. Then it must determine which way the eye is directed in relation to the monocle. From this you can deduce where the eye is pointed toward the screen. Alignment would be very simple, you put two crosses on the screen, one tracking purely the monocle, the other the reference as determined from the eye tracking. You then either adjust the monocle or the reference offset to stack the crosses.

If this can be done accurately you can eliminate the mouse.
