Basically, your format looks good to me. My input:

1. Change IDENTIFYING INFORMATION to BUSINESS SUMMARY

2. Eliminate COMPANY PERFORMACE, but put balance sheet, debt ratio, revenues, margins, in VALUE MEASUREMENTS

Remember, what we are trying to do is open a discussion and gather input from the Round Table. The format should be simple enough so we are inclined to post, but extensive enough to show that we have done DD before posting a new stock.
