I disagree regarding doctors switching from Premarin. Your whole premise seems to indicate that doctors are ultimately liable if they go to a new drug. This isn't true. Once a drug has been tested, deemed to be safe and approved by FDA, liability is very limited. If a doctor uses it outside of its intended use, there could be a problem though. Consider a more realistic scenario where a patient is prescribed Premarin and returns to the doctor a week later because of side effects. A good physician will probably consider an alternative and educate the patient about it.
