Some answers to your questions:

Servers, - no secret. Direct entry upon:
datek.com This site is not complete but lists the servers 1 through 15.

They had major outages at the day after thanksgiving and some days after - in Dec.98. Afterwards, they improved. Recently there were flaws in the early hours, but in Jan through March they were very good.

It is possible to trade at the open and some min. before. But it is more like an auction. Cancellations are very difficult at this time.

Hmm, an Island order crossing the market (eg a bid higher than the highest ask). I think you would get a fill at your bid by an MM if nothing is on the ISLD book on the ask side. You would get a better fill if you hit a lower ISLD ask with your order.
