hmmm.....I filled out the direct transfer....."in kind" forms...the 17 page package...for the "ultimate trader"...acct.. (a kit) and the account transfer form.....from their site..."Account Transfer Form" not sure if it is the one you mention... will check and see which is which...thanks...for the tip.. soon to be "ultimate trader Mary"..lol...
