Yes, you will need a separate modem, but this will be supplied by your new ISP. Though by the end of this year people will start seeing ADSL Modems on the store shelves.
Yes you need a different service, but it will probably be a familiar face "Bell Atlantic", and the Service is called Infospeed.
How much does it cost?
The ISP packages are, according to Bell Atlantic in Oct. 98.
Infospeed DSL 640K-bit-per-second for $40;
Infospeed DSL 1.6M-bps for $60;
Infospeed DSL 7.1M-bps for $110

The minimum PC system requirements for Infospeed DSL at present are:
Windows 95/98 or NT 4.0
Intel Pentium/Pentium Pro/Pentium MMX/Pentium II, AMD K5/K6, or Cyrix 586/686 processor
32MB RAM (64MB recommended)
25MB free hard drive space
resources to accommodate an Ethernet card if one is
not already present (available slot, available IRQ, available I/O resources)
The latest info I have is the service should be available in NY City in June! How's that for Timing!
