The narcotics are something of a hobby of mine. I don't much like methadone, even though it is long-acting and orally available. It has a lot of side effects. Morphine is still the gold standard. For those who tolerate it, fentanyl (the Duragesic patch) is effective and convenient.
<edit> Nonselective Cox inhibitors are good for pain. The Cox 2 selectives are thought of primarily as anti-inflammatories with diminished analgesia. The singularly best NSAID analgesic is ketorolac (Toradol). But it is quite Cox 1 active, and it can't be used chronically because of the severe GI side effects. Like bleeding ulcers.
