Before getting too excited about the Swedish study results, two things must be kept in mind;
1) It relied upon 'soft' behavioral measures, and the FDA does not like this kind of data. They prefer mortality (easy to measure, good interrater reliability) but in this instance, will insist upon standard cognitive measures which are more 'objective.' One can make a solid case for why behavioral measures are more meaningful, but....the ongoing studies do include such cognitive instruments.
2) The Alzheimer's target is not that important to NTI. Merz gets to keep 92% of the royalties paid by a marketing partner for AD...the neuropathic pain and AIDS-dementia targets are smaller in terms of market size, but NTI gets 75% of the royalties on net sales, paying 12% of that to Children's. To me, it is the lack of side effects that is most impressive, especially in an elderly population. That raises optimism regarding the safety profile in NP and AIDS-D--and as Dr. de Castro has noted, memantine comes from a class of drugs that has been plagued by adverse side effects (vacuolization, psychotomimetic effects, catatonia) to the point of being 'blackballed' by many Big Pharmas. To provide neuroprotective and analgesic effects without such side effects is crucial--and thus far the safety profile looks good.
