Heparin is used both prophylactically and therapeutically.

When it is used prophylactically, it is used in smaller doses before and after surgery and frequently replaced with coumadin orally. When heparin is used for treatment of blood clotts, it is used in much higher doses and for several days. This is when thrombocytopenia appears. Heparin is also used in smaller doses to keep intra arterial or venous lines patent, usually for several days and weeks and in patients on dialysis . This is when Heparin induced low platelets counts show up in small % of cases after several days of use.
