There's some specific form on which you declare that you're not subject to any backup witholding taxes, I think. I don't remember the form number, but remember there being something like that in the packet of paperwork that Wit sent me.

I had this withholding problem as well; it was solved once I sent back a copy of my app.

I got my checks about 3 weeks after sending the signature card.
