Just requested my certificate from E*Trade. If you use them, it is a piece of cake.

1) Click on "Customer Service" (top right)

2) Click on "Request Certificate" (on the list of choices, in the second column, near the top)

3) Fill out the form (Name, Phone, Email, number of shares & what company you are requesting certificate for), and send.

I don't think you have to talk to anyone, or try to tell them about Corporate Stock Transfer, Inc. As long as the request gets to BETT's transfer agent by June 30, your name should make the list of "registered shareholders" (SCOOBEY, can you confirm this for everyone?). Per the press release, your certificate does not have to be presented to BETT until September to get your dividend, so there is plenty of time for a certificate to be issued and mailed to you (contrary to whining over on RB).
