No, that was the money market clearing form you filled out with the Account name (your name) Account Number (From Step 11), SSN/TIN and the multiple signatures (I just had one).
There is no additional W9 form required.
I saw nothing that needed to be signed on the Account Agreement (21 pages).
I am sending 1) 6 pages of Account app, 2) Money Fund Clearing form, and 3) 2K check.
Agree?

I use W-9 form"s on a few of my DRIP accounts, if anyone has a blank copy he or she could fax it or scan it, and send any and all who need one.
