It should work the same way as a house ground fault. It would have to be located right at the battery pack for full protection. It detects a current from the hot to ground, in this case through a person, and immediately cuts out that wire's connection, until reset. It would be more difficult in a car because it's ground is not necessarily the same as earth ground, but I think that it can be solved.

More foolproof insulation at the connections would also make it pretty safe. There would also probably have to be some sort of crash cutoff device. That could be done with a momentum switch.
