The problem is there is a huge inequity in the private payer system and there really isn't any way to overcome it without having everybody pay or creating some sort of legislation around those lines. For example Dell contracted with some insurers in the Austin area for some absurdly low rates, which are in effect subsidized by the individual subscribers of the same plan. So it isn't just medicare that skews the system. If this were any other thing bought and sold on the open market well ok, but a basic necessity such as healthcare should not benefit one group at the expense of the other.
