For one man's perspective on how open source development works, you might check out Eric Raymond's (yes, another member of the Eric Hacker Conspiracy) The Cathedral and the Bazaar at tuxedo.org Eric is one of the most outspoken proponents of the Open Source movement - this article is what prompted Netscape to open the source to their browser. It's a long piece, but it stands as one of the most oft-cited works on open source.

Regarding coordination of open source projects, are you familiar with Cyclic Software's CVS version control system? It's another piece of software that I'd chalk up to open source success, and it's also a great tool for the community. Check out some of the web-based source browsers at cvs.gnome.org, for example.
