CBs, Bullion Banks, etc.

Does anyone understand the relationship between the two? For example:

1. Lease rates are quoted, as are forward rates - are these the rates charged by CBs to Bullion Banks acting as intermediaries?

2. If so, do the BBs then lease the gold to producers, hedge funds etc., and do they charge more? In other words, what's in it for them?

3. Do bullion banks sell direct to the market - in the same way as hedge funds, for example?

4. Who are the bullion banks - a few expmples would be interesting?

These are just a few questions - but it would be good to get some more information on this topic.
