Its true that Revelation is filled with metaphors but this particular sentence certainly can only mean on thing. It says, "a third of mankind will be "killed" and states, "The number of the mounted troops was two hundred million. I heard their number." I am only sure of this because it is the only place in Revelation where the author affirms that he is sure of the number. I'm not sure what other translation you could get out of this but I guess I would be happy to hear any possible other meaning.
