the thing that is the mystery to me is why the pilots didn't get an alarm of loss of pressure and reach quickly for emergency oxygen.

the usual thing would be for the pilots to use the emergency oxygen and quickly descend into altitudes where the passengers could breathe. typically there is not emergency oxygen for passengers but there could have been.
