I've actually seen fill-in-the-blank forms used for several (well, now they are) large public companies here in the valley. About the only thing missing is a glossary: 1)Insert stupid e.idea here 2)Get VC from kleiner perkins 3)IPO 4)Retire before the public realizes they've been scammed.
