Yes, if you see it, it is real. Though they MAY have the ability to hide the rest of the order like Instinet. I have a button on my TRADECAST software that automatically preferences the ECN showing the largest size first. If Island is on the inside I will use Fill or Kill of course and then use the ECN pref key for the remainder if other ECN 's are left. All ECN's are black color coded on my Level 2. This tells me immediately how to handle the order.
On the pre-market question... NO, Sometimes they are prices left up from previous session and you cannot hit them. Though usually you will see them update and then they are real. The other day in the aftermarket I got the word from the Bloomberg pit that a stock beat the street by about 40% and I was able to pick off MWSE for 500 shares. Made a 9 point gap in the premarket!!!
