There is a good chance that the cable is either seated improperly or reversed at either the mobo or floppy. I got the same error when putting together my system and fixed it by flipping the cable at the rear of the floppy.

Holy Shit! I had no idea there were so many possibilities for a floppy disk to fail. I can't imagine getting 'failure 40', since I'd be totally nuts before I got to 'failure 10'.

I agree with the other guy, take a look at your cables first, jumpers next, and BIOS as a last resort.
