The program that keeps coming back...

I'm assuming you took the thing out of your Startup folder on Start menu.

Other places to check:

Commands in .INI files, such as SYSTEM.INI, can be detected by using Find command.

Search registry and see if the program is being invoked from there.

Perhaps the toughest one, but fortunately the rarest, is if the program is being started by another program. Use System Information to selectively start your system. Keep track of what's starting and where the unwanted guest appears and you'll soon know what's misbehaving.
