Well the reason I asked about Couch is that I have read about the notorious twiggy drive as if it was instrumental in Lisa's problems... I know it had a giant failure rate and with 10meg hard drives you really needed a floppy. I don't know the history of the twiggy, (it was high volume floppy media drive), but I was under the impression it was designed and built by Apple ... the NIH thing, vs. the preferred method of OEM from sony which is what they eventually did. It just seems like a junior manufacturing team will say, 'ok lets go with a single source home grown peripheral'... experienced manufacturing people know not to do that... but who knows what the conventional wisdom was back then - (wonder where they got the mouse?)
