A suggestion for you... Think "star of stars" topology...

Look around your neighborhood and you will probably find a box that terminates a big black wire bundle cable. This cable runs back to your CO. In my case the run is 6.1 miles - too far for DSL, but...

An enterprising outfit will realize that a small concentrator box can reside in the terminal box. This box will be the source of RADSL for 10 to 100 customers. This box should cost below $200 per line served.

The box will mux these customers on to one or a few parallel twisted pair using T1 type provisioned standards for the back haul to the physical CO. This is "old hat" for telcos. The distance is manageable for lines that are selected and tweaked. I believe this is economic to serve as few as ten customers per backhaul.

Now you need to present this solution to your local providers and ask why they can't do it for you while strongly suggesting that your intent is to find somebody that will :-)
