A registered rep would call the firm for you; upon finding out that there was more to go, he'd get the applicable trader's assistant on the phone, they'd give a price and size, and upon agreement would get ACT information, your MPID (and if your executing BD was introducing, the clearing firm's ACT, MPID, and possibly NSCC or DTC numbers? can't remember) and by the end of the day, you'd have a print. Usually, compliance officers' names and numbers are swapped too.

Yes, this is why SelectNet pref. is the preferred method :)
