A better scenario would be if someone pushed her into the back seat of the car and then he and maybe an accomplice forced her face down onto the floor. In this position it would be very easy to stab her in the back and head.

I thought about this and it does make sense, but there are two things that bother me: a) her back would have been accessible, and thus a much bigger target, yet all the wounds were to the head and neck, and b) this makes the killing more execution like, IMO, as she'd be face down. I envision a conversation in which she was either uncooperative of just said the wrong thing and was killed in a fit of rage. On the other hand, if a pen knife were indeed used, maybe the killer had no choice but to aim for an exposed area, which would have ruled out any area covered by her jacket.

As for restraining someone from the rear, I don't mean physically wrapping one's arms around the victim's neck while the car was in motion, I just mean holding a knife close (i.e. within striking distance). Does this change your thinking? What probability would you assign to either scenario?

Yes. That makes lots more sense than the front seat theory.
