sec.gov here is also one "Frank Valdez" mentioned but without residential address. Look for "valdez" in that document. A form 4 (Stock Sale) is mentioned to have been filed subsequently but it is not on the SEC database. Therefore it is difficult to retrieve the addresses... The Company is aware that during 1997, each of Frank Corella, Frank Valdez and Giulio Perillo failed to timely report one transaction on Form 4, which transactions were subsequently reported.
