I was more familiar with IRMX and VRTX all of which are obsolete now. Still the last project I was on was high volume, low cost. After looking at OSs it was decided that we could save a nickle per device by doing our own and so we did.

That is exactly the analysis most companies do, and it is the reason 85% of the market is still roll your own. Unfortunately for many companies, is the fact that it is much easier to do with a 8 or 16 bit processor doing a couple of tasks than it is for a 32 bit processor responsible for multiple tasks, including communicating with the outside world.

More and more, time to market is a controlling factor. If a company can get a product to market 6 months faster by outsourcing the operating system, it often makes sense, even if it costs a couple of dollars extra per product.
