FWIW, I used to work for an institution that held shares in various companies and noticed that even large holdings we had often did not show up on those types of public reports. So I think that there is frequently under-reporting of institutional holdings and you should be aware of that. There were a large number of institutions who received shares of Biomira a few years ago as a result of Altamira Capital's spin out of shares to their shareholders through a butterfly transaction (tax efficient method). So I think Biomira has more institutions with holdings than are reported, particularly Canadian institutions.
