Fiber to the home would enable the 100% solution and there are companies that are beginning this effort though it is a ways off. I say take that 90% solution and begin to leverage it by creating a service offering for content creators. Facilitate the creation of true BB apps by providing the mechanism within which they can be BB entrepreneurs. ATHM solves two issues and they are home free. Solve the the uncertainty at the bone level by productizing BB content positioning with respect to the bone. Solve last mile by setting metrics that address LAN congestion in the HFC model and set metrics for the DSL, satellite, tin can models. Become last mile agnostic by setting the rules for @Home optimized and let them come to you.
