In my case, I didn't lose the card. Someone got the number somehow. I called the 800 number 3 or 4 times, and each time I was told the magic form was going to be sent in "a week or so".

I never did get the form. On my last call, I finally got an explanation on how I can create my own 'form' and where to fax it to. I finally got credit for the false charges, but it was an ordeal.
