If there is a market for the option itself...then the option has value in addition to the difference between the strike and the current underlying security price...In the U.S. the options market is supported by market makers and guaranteed by a clearing organization...You just sell the option and realize the full value of the option...

Now...selling a long call and then buying the underlying security...is two separate transactions...but if there is a loss on the option then it could be a wash loss...
