The rules for applying to NASD for a SmallCap Market listing or National Market listing (the criteria are more stringent for a National Market listing may be found at:

nasdaq.com

The key criteria is a minimum bid price of US$4 (for the
National Market the minimum is US$5), which must be maintained for 21
consecutive trading days. Once application has been made, the share price
must also be maintained during the 8 week review period

I think it has to trade for an avg of 4.00 US for 30 days. The occasional intra day dip does not count.
So add up the last 30 days and what do you get. Most companies wont list until it achieves consistent range over 4.00.
The calculation is based on closing prices. Anyone please feel free to correct me.
