Utility theory SUPPORTS fairness; they're not at all inconsistent. It's just a question of what IS fair? Is it fairer for everybody to give the same percentage of DOLLARS, or of PAIN (given that the two aren't linearly related)? Your "bottom-end cutoff" is just another way of recognizing the same effect (unless you simply think it's more efficient from a tax collection standpoint to not bother with the minimal dollars available at the low end). It's the old "you can't get blood from a turnip" thing.
