I understand your predicament. I've had the fortune to ask many of the questions you have to a recently retired loop engineer from PacBell.

I can't answer your questions with authority, but perhaps may be able to shed some light. You ask a good question as to physical treatment of the copper end, & really don't know the answer other than the copper upon entering to CO likely connects to some device from which the physical line may extended (spliced) to connect to whatever switch necessary to provision the line.

As far as provisioning the line itself, in terms of eliminating bridge taps and any load coils, you ask another good question. On one hand it is likely the CLEC's responsibility for ultimately provisioning the line so that the subscribed service can be delivered to the customer. On the other hand, I can see how the incumbents may not wish CLEC personnel to gain unfettered access to loops in the binder. The customer premise and the CO is one matter, opening a vault or conduit housing the binder where there are bundled copper loops not within the purview of the CLEC is something I think the incumbents would be loathe allow, at least without supervision or oversight, which is costly. Most likely, the CLEC must commission the ILEC to provision these aspects of the loop before the CLEC may provision the terminal connections.

I emphasize this is just speculation on my part, but a rational one. I see if I can get better info for you, one that has some authority behind it, unless there are some hard core teleco guys out there than resolve the issue in quick fashion.
