I add a supplemental page listing the open shorts for the year from each broker and show how the total if added to cap gains report total sales column equals the 1099-B amount. I don't think you have to do that but anything to make things clearer, I say. Otherwise, you may end up getting a request for more information anyway.

I attach a note to the D-1 with the trade date, stock name, and proceeds. Example: "Note: Short Sale of 200 IRID from 6/30/99 open as of year end. Total proceeds from sale were $2190.82".
