It doesn't necessarily have to be management, if everybody is clear on what's acceptable and what's not. Of course, if a cavalier attitude pervades the place, maybe each line should be independently audited.

I've worked in code development. The production code we shipped was seen by multiple eyes. Of course, our customers had copies, too -- maybe that's what is needed here. <g>
