An unknown small biotech firm would have a difficult time initially in promoting a new product.One reason: the difficulty the reps would have in seeing physicians for the first time when they represent the unknown and unfamiliar. That is of course completely overcome when someone representing a company like J & J calls.They have the connections and the experience to call upon those most likely to prescribe right off the bat.
The exception would be a flood of media exposure for a new medication (e.g.like QLT) that pretty well forces the physicians to learn all about it quickly.
