John,

You write: Take this for what it's worth, but in my view, it all starts with Kathy Levinson. Her ineptness trickles down to the entire company, from the kind of people they hire, to their training, to their attitude toward customers.

I am curious that you place the blame on one person, specifically Levinson. The problem I have with the above statement is that it is so darned general. What specifically does Levinson do or not do that leads to this spiralling disintegration from within? Or, to put it another way, describing someone as inept is usually to point to certain actions. Care to elaborate which of her actions have led you to that conclusion?

I certainly would appreciate any insight into the workings of the company that I have not already considered.

Thanks, and good luck
